Responsibilities
|
|
The Physical Therapist is a licensed professional who provides direct and indirect patient care and restoration of normal body functions after illness or injury. The Physical Therapist provides physical therapy services in accordance with Agency policy and governing regulations.
|
|
Qualifications
|
|
ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
- Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:
A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
B. Assessing for muscle strength, mobility, gait, ROM¿potential for rehab.
C. Directing physical therapy treatment.
D. Instructing patients and families/caregivers in the use and care of therapeutic appliances.
E. Determining priority needs for physical therapy.
F. Reporting to physician patient¿s reaction to treatment or changes in condition.
- Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other Agency professionals.
- The Physical Therapist may be requested to visit patients that live outside of the community gates. This would require driving own vehicle or using public transportation to client location. (FRV only)
- Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient.
- Trains patient in the use of prosthetic device.
- Communicates with the physician regarding patient¿s needs and reports changes in the patient¿s condition; obtains/receives physician¿s orders as required.
- Communicates with other Agency personnel to ensure coordinated and comprehensive care.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Provides physical therapy consultation to home families/caregivers when indicated.
- Supervises physical therapy assistants according to Agency policy and or State regulations.
- When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks per State regulations.
- Reports all client complaints and incidents; implements appropriate corrective action as directed and follow-up when necessary.
- Works with other departments to improve the customer service, communication and care management.
- Maintains productivity, as appropriate.
- Participates in case conferences with the Clinical Manager by reviewing: services, clinical documentation and discussing pertinent patient care issues and goals. Implements suggestions of Clinical Manager to promote more effective performance and delivery of quality services.
- Participates in quarterly record reviews and communicates findings and recommendations to appropriate Agency personnel. Participates in quality performance activities.
- Submits an accurate record of visit/hours worked each day.
- Submits accurate, complete, timely, meaningful, and required documentation.
- Maintains and protects the confidentiality of patient information at all times, enforces patient rights regarding privacy, personal property, and grievances.
- Assists with orientation of new staff as requested.
- Attends in-service and continuing education programs.
- Complies with acceptable professional standards and principles.
- Works with other departments to improve customer service, communication and care management.
- Complies with local, State and Federal laws regarding licensure and certification of Agency and maintains compliance with accrediting body, if applicable.
- Maintains department philosophy, goals and objectives, standards, policies and procedures, and interprets these to ensure high-quality care.
QUALIFICATIONS/EDUCATION/EXPERIENCE:
¿ Degree from a baccalaureate or masters program in physical therapy approved by an accredited organization or as required by State regulations.
¿ Current license to practice as physical therapist within the State.
¿ Two years of appropriate experience as a physical therapist or as required by State regulations. Community/home health experience is preferred.
¿ Current CPR certification.
¿ Good organization skills.
¿ Employees providing care to patients that live outside of the community gates will be required to present a valid driver¿s license and drive or use public transit to client location. (FRV only)
LANGUAGE SKILLS:
Good written skill; must be able to document clearly to meet reimbursement requirements. Must speak, read and write the English language. Good verbal communication skills

OSHA OCCUPATIONAL EXPOSURE CATEGORY:
After careful analysis, it has been determined that this position falls into OSHA Occupational Exposure Category A (Category A in ERC Policy) requiring the Hepatitis B vaccine series and the use of protective equipment.
