Responsibilities
|
|
SUMMARY: A Home Support Aide is a qualified individual who carries out social, personal and certain health care tasks under the supervision of a registered nurse and who may also provide assistance with housekeeping and other related supportive tasks to a client in his/her place of residence.
ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
1. Performs routine Home Support Aide Functions, including the following:
Assists with care of teeth and mouth.
Assists with grooming care of hair including shampoo, shaving and the ordinary care of nails.
Assists with bathing of client, in bed, in tub and in shower.
Assists client on and off bedpan, commode and toilet.
Assists client in ambulation or transports client by wheelchair, as appropriate.
Assists client with eating.
Assists client with dressing.
Arranges the schedule, with guidance from the supervisor, so that client follows medical recommendations such as increased physical activity.
Takes temperature, pulse and respiration when instructed by supervisor.
Keeps simple records as instructed by supervisor.
Measures intake and output as instructed by supervisor.
Observes, reports, and records client activity as instructed by supervisor.
Responds to emergencies and practices safety and accident prevention while on duty.
Observes infection control and standard precautions.
Observes client rights and confidentiality.
Uses prescribed medical equipment and supplies.
Accompanies client to obtain medical care, as indicated.
Prepares and serves meals according to instructions.
Keeps the rooms the client uses clean, including making bed, changing bed linens, dusting, vacuuming, washing dishes, tidying kitchen, bedroom, and bathroom.
Lists needed supplies.
Shops or does errands for client.
Washes client's laundry.
Participates in Performance Improvement activities.
Documents services provided on the appropriate form(s).
Prepares and submits activity records and hours/visits worked.
Attend in-services, as required.
2. Performs the following permissible Home Support Aide functions under special circumstances:
Assists with changes of colostomy bag.
Changes simple non-sterile dressing.
Assists with the use of devices used as aids to daily living for the disabled.
Assists client with prescribed exercises about which the home support aide has been taught by appropriate professional personnel.
Assists with skin care as directed.
Performs simple urine test for sugar, acetone or albumen and record results.
Prepares modified diets, as prescribed by physician.
Helps client relearn household skills.
Assists with payment of bills.
Performs other tasks permissible under state scope of practice and regulatory guidelines under the direct supervision of a Registered Nurse.
Observes Advance Directives status as directed on each individual client. Home Support Aide Assists with medications.
3.Performs other duties as assigned by Supervisor.

Qualifications
|
|
LICENSES, CERTIFICATES, REGISTRATIONS:
Certified as a Home Health Aide, Certified Nursing Assistant, Geriatric Nursing Assistant or Personal Care Aide, or as per State and Federal law and regulations.

Demonstrates maturity and mental and emotional stability.
Able to read and write English in order to understand and carry out directions and instructions and maintain simple records.
Has recent experience in personal care and homemaking.
Demonstrates a willingness to learn and a basic understanding of the duties of a home support aide and the needs of the client.
Able to accept and use supervision.
PHYSICAL DEMANDS:

Must be able to operate medical equipment, as instructed.
Must be able to climb stairs.
May be required to lift, pull or roll 50 pounds.
Must be able to lift and transfer clients.
Must be able to perform routine household duties, which may involve bending, reaching, and lifting.
Must provide proof of non-infection to tuberculosis.

Frequent contact with residents and families.
Frequent contact with various groups of health care workers.
